Some places tuft using upholstery twine attached randomly while others may tuft using metal rods or a combination of the two.  Both are effective, it is up to the upholsterer. The buttons must be attached well to avoid popping off with use.  Each time you sit on a tufted piece of furniture, the strength of the tuft is challenged.  Thus, the tuft must be placed securely.

This traditional art is now showing up in more transitional or modern looks in leather furniture.  Button tufted leather sofas are now common in coastal homes as well as mountain cabins.  Once thought to be very firm to sit but today's new softer leathers take a tufted piece of furniture to a new level of comfort.

Softer leather makes for a softer seat.  Those that still want the firmer sit on a tufted chair can still get that by selecting a stiffer, firmer leather.  Heavyweight leathers are a good choice for a firmer sit.  However, some are so thick it makes them hard to use for tufting furniture.  The thicker the hide the harder it is to skive the leather to make the button tufts.  The thickness of thick leathers must be reduced or skived to allow the buttons to be made and then applied to the leather furniture.
